About DHC
I’m Deborah Hawthorne, a rural pharmacist and credentialled diabetes educator, and the founder of DHC. I provide clear, practical, and compassionate diabetes education for people at every stage of their journey.
Based in Wangaratta and Benalla, I offer face to face clinics as well as telehealth appointments across Australia, ensuring people in rural and regional communities can access the support they deserve.
My work focuses on empowering you to understand your condition, build confidence, and make sustainable changes that fit your life. With a strong background in medication management and chronic disease care, I bring a holistic, real world approach to diabetes support.
Services
Diabetes Education – Initial Visit
A 45–60 minute personalised appointment focused on understanding you — not just your numbers.
We explore your lifestyle, routines, goals, concerns, medications, and glucose monitoring. Together, we create a clear and practical plan to support your confidence and next steps.
Continuous Glucose Monitoring (CGM)
A 30–45 minute hands on session designed to help you feel confident using CGM in everyday life.
Includes a complimentary sensor (valued at $60–80), setup support, and guidance on understanding glucose patterns, alerts, and trends.
Diabetes Education Review
A 20–30 minute check in to review what’s working, what’s changed, and any challenges.
Your plan is updated to stay practical and achievable.
Home Medication Review (HMR)
With a GP referral, I assess safety, simplify routines, and provide clear guidance, with a detailed report sent to your GP.
Federally funded — no out of pocket cost
DHC works collaboratively with GPs, nurses, and allied health professionals to support patients living with diabetes.
Services complement care plans and focus on practical education, medication understanding, and building patient confidence in self management.
