A Home Medication Review is a comprehensive service designed to help you get the most from your medicines.

Deborah visits you in your home to review prescriptions, over‑the‑counter products, and supplements. Together, you’ll identify concerns, improve safety, and simplify your routine.

A detailed report is provided to your GP to support better health outcomes.

A doctor is required to refer for a HMR, so speak with them to discuss your eligibility today

How can you get the most out of your medication review

  • Ensure all of your medicines (tablets, creams, puffers, etc.) and over-the-counter supplements are available

  • Have your medication list accessible

  • Have any related pathology or hospital discharge information also present

Benefits of a home medication review can include:

About Deborah

Deborah Hawthorne is a rural pharmacist and credentialled diabetes educator with extensive experience in medication management and chronic disease care.

She works closely with patients and GPs to improve understanding, simplify treatment plans, and support safer, more effective use of medicines.

Her approach is practical, person‑centred, and grounded in real‑world care.

A detailed report is provided after each review to support clinical decision‑making.

How To Get A Referral

STEP 1

Book an appointment with your GP

STEP 2

Ask about a Home Medication Review

STEP 3

GP determines suitability for home medication review

STEP 4

GP sends referral to deborahhawthorneconsultancy@gmail.com‍ ‍

STEP 5

Contact will be made to set up an appointment in your home

STEP 6

Review is completed (generally takes 60 minutes)

STEP 7

Report from appointment is sent to GP

STEP 8

GP will discuss results at next appointment

This service is a free government funded service for Medicare card holders
