We explore what life looks like, goals, worries, routines, and what you feel ready to work on. Together we review medications, glucose monitoring, and any immediate challenges, then create a simple, personalised plan that supports confidence, safety, and next steps in their diabetes journey.
The CGM Application & Skills Session is a 30–45 minute hands‑on appointment that supports clients to feel confident using continuous glucose monitoring in their everyday life. Together we apply a complimentary CGM sensor (valued at $60–80), talk through comfort, placement, and wear tips, and set up their app or reader in a way that suits their routines. Clients learn how to understand their glucose patterns, alerts, and trends, and leave with practical, personalised strategies to make the most of their data. This session is ideal for anyone wanting to trial CGM or build confidence with their device.
This is a 20–30 minute check‑in designed to meet clients exactly where you are. We explore what’s been working, what’s changed, and what challenges or questions have come up since the last visit. Together we review glucose patterns, medications, routines, and goals, then refine plans to keep things practical, achievable, and aligned with their daily life. This appointment helps clients stay supported, confident, and on track with their diabetes management.
The Home Medicines Review is a comprehensive in‑home appointment where I sit down with clients to go through all of their medicines — not just diabetes treatments. With a GP referral, I visit the person in their own home, look at every prescription, over‑the‑counter product, supplement, and device they use, and explore how these fit into their daily routines. We talk about what’s working, what’s causing confusion or concern, and any safety issues or practical barriers. Clients receive clear, personalised guidance, and I provide the GP with a detailed report to support safer, simpler, and more effective medication use.

Based in Wangaratta and Benalla, I offer face‑to‑face clinics as well as telehealth appointments across Australia, ensuring people in rural and regional communities can access the support they deserve.
